All new Cub Scouts must complete Parts A and C of this form and submit it with their application to Pack 59's Membership Chair.  Returning Scouts must update the form each year when they re-register. 

All adults taking positions as Leaders, Assistant Leaders or other Leadership roles in Pack 59 must submit a New Adult Application to Pack 59's Committee Chair.
